Sports Paperwork
All athletes participating in athletics in the Central Kitsap School District must have all paperwork in before they will be able to practice. This includes a current physical, emergency card, Clearance Form, Athletic code and an insurance form specific for the sport being played. (please note multiple sports will include different insurance forms.) For your convenience we have these forms available online for you.
